PLACE: Grand Wayne Center, 120 West Jefferson Boulevard, Fort Wayne, Indiana. The Meet will be in a ballroom that is 9,000 square feet!

ENTRY FEE: $60.00 per lifter; $45.00 each additional division; $50.00 per team. Entry fee also includes meet T-shirt. Please indicate T-shirt size on entry form. Team roster and fee must be sent in by entry deadline. Any changes to team roster must be submitted to Meet Directors prior to Saturday weigh-ins. ENTRY FEE IS NONREFUNDABLE!
DEADLINE: ENTRIES MUST BE POSTMARKED BY DECEMBER 29, 2000. No late registration allowed.
TICKETS: Adult spectator fee is $10.00/Day, 2-Day for $15.00. Children (7-12) $3.00, children 6 and under free. ONLY TEAM COACHES WILL BE ADMITTED IN FREE. (max 2 coaches per team). Any additional coaches for a particular team will have to pay the above admission fee. Tickets can be purchased in advance with entry form. All lifters and coaches will be given a security pass that will be REQUIRED to enter the warm-up and lifting areas. This pass will need to be presented to the security personnel to enter these areas. This is being done to ensure the warm-up and lifting areas remain as comfortable and uncrowded as possible for the lifters.

ELIGIBILITY: Competition is open to all registered USA Powerlifting Women athletes who have met the Qualifying totals from January 1, 2000. USAPL cards can be purchased at the meet site. All lifters must be drug-free for thirty-six (36) months.

RULES: This meet is sanctioned by the USAPL. Current USAPL rules govern this contest.
DRUG TESTING: Urine. All lifters must willfully subject themselves to drug testing as required. Failure to do so will result in immediate disqualification from the meet, along with the forfeiture if any award earned.
USAPL IPF WOMEN'S WORLD TEAM SELECTION: This competition
is THE QUALIFIER for the 2001 IPF Women's World Powerlifting Championships.

MEET HOTEL: Fort Wayne Hilton, 1020 South Calhoun Street, Fort Wayne, Indiana, 46802. (219) 420-1100, FAX (219) 424-7775. A limited number of rooms have been reserved-call early!! Any rooms left after the January 2, 2001 deadline will be released to the general public. The discount rate of the rooms for this events is $75.00 for doubles. This hotel is attached to the Grand Wayne Center, the Meet Site.
Reservations received after January 2, 2001 will be confirmed based on space availability at the above rates plus $10.00 per room per night (hotel policy). Also, the Fort Wayne Hilton offers complimentary airport shuttle service for their guests. Please schedule their airport van in advance by calling the hotel. Transportation will be scheduled as close as possible to your arrival and departure. To minimize waiting time, please provide your airline flight number and time of arrival. In addition, a hotel courtesy phone is provided in the baggage pick-up area of the airport.

In consideration of the acceptance of my entry blank in this powerlifting competition I intend to be legally bound for not only myself but also for my heirs, my executors, and my administrators. In signing this release from liability I waive and release Allen County Family & Sports Chiropractic, Drs. Michael and Monique Hartle, the Grand Wayne Center, the Fort Wayne Hilton and sponsors and everyone connected with this competition from any and all liability, including any result of negligence which may arise from this competition.
Morever, I agree that any testing method which the meet director and the sponsors of this meet use to detect the presence of strength-inducing drugs SHALL BE CONCLUSIVE. That is, whether I think the results of the tests are right or wrong I agree that I have no right to challenge the results of the drug tests. I further agree to submit to any physical tests which may be necessary to complete the drug testing. Should I fail to pass the drug tests I agree to forfeit any trophy or award which I might otherwise have won. I understand and agree that if I fail to pass the drug tests, my name will appear on a published list of suspended members. If it is determined that I have failed the drug test I agree to waive any claim for which legal relief is available.
I agree to pay any attorney fees and litigation expense incurred
by any person, real or corporate, whom I may sue in an effort
to challenge this release from liability form. I understand that
my agreement to pay attorney fees and litigation expenses is the
sine qua non for the acceptance of my entry in this contest. If
any provision of this Release from Liability shall be deemed by
a court of competent jurisdiction to be invalid, the remainder
of this Release from Liability shall remain in full force and
